If you're running a listen server...
in console
removeid STEAM:ID:YOU:WANT_UNBANNED
writeid

If from a dedicated server, use its console following instructions above,, or rcon from your client
rcon removeid STEAM:ID:YOU:WANT_UNBANNED
rcon writeid
