User perm banned when server restarts

There is a user on the server I am management on who is constantly getting banned permanently when the server restarts. I have looked through the bans.txt file and have not found his ban. If someone could give me a solution to the problem that would be highly appreciated.

bans.txt from ULX is just where we keep temp bans, and info of who/why for all bans (temp and perm)
ULib still uses the default gmod ban file, which, unless Gmod13 changed it, only stores steamid and time in your server/gmod/cfg/banned_user.cfg file.
Source stores bans in that (and/or banned_ip)
