You have banned (Console) permanently (reason)

I asked this on Facepunch and they told me to post here, help!

Whenever I ban someone or my staff bans someone, the ban advert shows as ex: "burger has banned (Console) permanently (Evading ban, mass RDM)" why does this happen? Everyone on the server then goes like "WTF" "CONSOLE CEPTION" and I have no clue, it says in the advert that they ban console but when I look into the bans, they banned the correct player! This is weird and it's been happening for a while now in my DarkRP server, what could be happening?
